We are happy and excited that you are interested in taking part in Family-to-Family’s Home Essentials Project. This program is designed to help you with the basic essentials you will need as you move into temporary or permanent housing.

Your sponsoring family is hoping to help you for 3-6 months as you rebuild your lives, sending items such as sheets, towels, blankets, clothing, over the counter medications, baby care items, personal care items, small household appliances and whatever else you might need. In order for your sponsor to best help your family we need you to fill out the questionnaire.

Your sponsor will use this questionnaire as a guide so
they can assist you in the best way possible!

You are not required to give the full names of your family members, but you may if you would like to. We hope that you and your sponsor might develop a friendship through this process – but how involved you want to be is up to you.
